Another thing that strikes me is that the balloon is always displayed
SE of the mouse (OK, the SE would be fixable, but the problem is more
with the "always" than with the actual direction).

For modeline extents this is most often the wrong direction. it would
be better if the _lower_ left corner of the balloon frame would be
roughly at the height of the mouse.
